Please inform our office if you have any of the following medical situations:

• Artificial heart valves
• A history of infective endocarditis
• Congenital heart conditions, repaired or unrepaired
• A cardiac transplant that develops a heart valve problem
• Joint replacement surgery (i.e. hip, knee) in the past two years
• Are taking oral bisphosphonates (i.e. Boniva, Fosamax, Actonel) usually for osteoporosis
• Are taking blood thinning medicines (i.e. Coumadin, aspirin)
